Care Supreme is Care Health Insurance's flagship retail plan and the one that independent advisors consistently cite when recommending alternatives to HDFC ERGO Optima Secure at lower premium. It earns that positioning — the plan's feature set is genuinely competitive. But understanding Care Supreme requires going beyond the base plan to understand how the add-ons change the value proposition, and where the plan has real gaps.

The higher complaint volume is the ongoing watch item for Care Health. The improving CSR trend means claims are being settled at a stronger rate than before, but the process involves more friction than lower-complaint insurers.

Care Supreme Base Plan — Features Verified

No Room Rent Restriction

Care Supreme has no room rent cap — any room category is covered without triggering proportional deductions on the rest of the bill. Per Ditto's Care Supreme plan page: "no room-rent condition to argue about." This eliminates one of the most common sources of claim disappointment in lower-tier plans.

Unlimited Restoration — Same and Different Illness

Built into the base plan: the sum insured restores unlimited times for both related and unrelated illnesses. Per Policybazaar's Care Supreme page: "With this benefit, the policy sum insured is recharged unlimited times in a year for the same as well as unrelated illnesses in case the base sum insured gets exhausted due to one or more claims."

This positions Care Supreme alongside Niva Bupa ReAssure 2.0 as one of the only mainstream plans offering same-illness unlimited restoration. HDFC ERGO Optima Secure's base plan doesn't offer same-illness restoration; Star Health Family Health Optima only restores 3 times for different illness.

No Co-Payment

No co-payment on any claim — per Ditto's Care Supreme review (June 2026).

Pre and Post Hospitalisation

  • Pre-hospitalisation: 60 days — per Ditto's plan page
  • Post-hospitalisation: 180 days — double the 90-day standard at most competing plans

Cumulative Bonus

Standard NCB: 50% of SI per claim-free year, up to 100% maximum — per Policybazaar.

With the Cumulative Bonus Super add-on: up to 500% of the base sum insured accumulated over claim-free years — per Ditto's Care Supreme article (June 2026). A ₹10 lakh base policy can grow to ₹60 lakh effective coverage over time with this add-on. This is one of the most aggressive cumulative bonus structures in the market.

E-Consultations — Unlimited, In Base Plan

Unlimited e-consultations with general physicians within Care's network are included in the base plan — per Policybazaar. This provides limited outpatient coverage without requiring the OPD add-on.

Women's Care — OPD for Women's Diagnostics

Per Investkraft.com's Care Supreme listing: covers diagnostic services like mammography, PCOS/PCOD tests, and cervical cancer screening up to ₹50,000 per year — a specific feature for women policyholders.

Air Ambulance

Emergency air ambulance services covered up to ₹5 lakh per policy year — per Ditto's Care Supreme review (June 2026).

The Add-Ons That Change the Value Equation

Care Supreme's base plan is strong. The add-ons make it significantly more comprehensive. The most important:

Instant Cover — Day-31 PED Coverage (The Standout Add-On)

This is Care Supreme's most discussed differentiator. The Instant Cover add-on covers specific pre-existing lifestyle conditions from Day 31 of the policy — not after the standard 36-month PED wait.

There are two Instant Cover variants:

  • Basic Instant Cover: 4 conditions (diabetes, hypertension, asthma, hyperlipidaemia) — from Day 31
  • Advanced Instant Cover: extends to broader conditions including obesity, COPD, and coronary artery disease (PTCA done over a year ago) — also from Day 31, per Ditto's June 2026 review

This competes directly with Aditya Birla Activ One MAX's Chronic Care add-on (7 conditions from Day 1) and ICICI Lombard Elevate's Jumpstart (from Day 30). Care Supreme's variant covers 4–6+ conditions from Day 31 — a competitive alternative.

Claim Shield Plus — Consumables Coverage

Per Ditto's Care Supreme plan page: this add-on covers non-medical consumables (gloves, PPE, masks). Not included in the base plan — compare with HDFC ERGO Optima Secure, which includes consumables in the base plan.

Wellness Discount — Up to 30% on Renewal

Per Ditto's article (June 2026) and Care Health's official page: walking 10,000+ steps daily and meeting wellness criteria earns up to 30% discount on renewal premium.

This is comparable to Aditya Birla's HealthReturns (up to 100% back for very active buyers) but more achievable for moderately active policyholders. A 30% discount on renewal premium is genuinely meaningful — on a ₹25,000/year plan, that's ₹7,500 saved annually.

OPD Coverage — BeFit Add-On

Ditto's Care Supreme review (June 2026) references: "8 outpatient consultations annually, 4 with general physicians and 4 with specialists, each capped at ₹500 per visit." An OPD add-on is available; the base plan includes unlimited e-consultations but not standard OPD pharmacy/diagnostics.

There's also a gym access add-on — unlimited gym visits — per Ditto.

PED Wait Period Reduction Add-Ons

Beyond Instant Cover (which specifically targets 4–6 lifestyle conditions), separate add-ons reduce the general PED waiting period from 3 years to 2 years or 1 year — per Ditto's Care Supreme review. These apply to all pre-existing conditions, not just the Instant Cover list.

Premium — Verified for Noida/NCR Zone

Per Business Standard's premium comparison (January 2026, citing Policybazaar data): Care Supreme at ₹27,276/year for a ₹10 lakh SI, standard profile — vs HDFC ERGO Optima Secure at ₹39,268/year for the same profile.

Care Supreme is approximately 31% cheaper than Optima Secure at this profile. With the key add-ons (Claim Shield Plus, Cumulative Bonus Super, Instant Cover, Annual Health Checkup), per Ditto's article (June 2026): the all-add-ons premium in Delhi is noted as illustrative at ₹15 lakh SI.

What Care Supreme Doesn't Include

No maternity coverage — per Ditto's article (June 2026): "Care Supreme does not offer maternity coverage, even as an add-on." Families planning children must look elsewhere — Niva Bupa Aspire, HDFC ERGO Optima Secure with maternity add-on, or Aditya Birla Activ Fit.

Consumables require a paid add-on (Claim Shield Plus) — not in base plan.

OPD coverage limited in base plan — the e-consultation benefit covers physician access but not pharmacy or diagnostics without an add-on.

Who Should Look Elsewhere

  • Families planning to have children — maternity simply isn't available in Care Supreme
  • Buyers who want the smoothest claim experience — the higher complaint volume means claim support matters; HDFC ERGO or Aditya Birla Activ One MAX are cleaner on this metric
  • Those who want consumables in the base plan — HDFC ERGO Optima Secure includes these without an add-on
