ICICI Lombard Health Insurance Review 2025–26 — Elevate Plan, Claims, and What to Know

ICICI Lombard General Insurance Company Limited is India's largest private general insurer by premium. It's backed by ICICI Bank, one of India's largest private banks, and has expanded rapidly in retail health insurance in recent years — its retail health market share grew from 3.3% in FY 2025 to 4.1% in FY 2026, a 54% growth in the total health portfolio per Ditto Insurance's ICICI Lombard review (September 2026).
But growth in market share and reliability in claims settlement are different things. Here's what the verified data shows.
Company Background
ICICI Lombard started as a joint venture between ICICI Bank and Fairfax Financial Holdings (Canada). With ICICI Bank as its primary promoter today, it operates as a general insurer — health, motor, travel, commercial, and home insurance under one portfolio. It does not operate as a standalone health insurer.
Business scale per Ditto Insurance's review (September 2026):
- Gross Written Premium (health): ₹7,955 crore — nearly double the industry average for private health insurers
- Network hospitals: 9,500+ — per ICICI Lombard's official Elevate page
- Claims processing uses TPAs (Third-Party Administrators) — not in-house, unlike HDFC ERGO, Aditya Birla, and standalone health insurers
Claim Settlement — The Key Numbers
Claim Settlement Ratio (CSR)
This is where potential ICICI Lombard buyers need to pay careful attention. Per Ditto Insurance's ICICI Lombard CSR analysis (July 2026), using consistent NL-37 IRDAI filing methodology:
- CSR FY 2024–25: 83.65%
- 3-year average CSR (FY 2022–25): 84.50%
- FY 2024–26 average: 86.81% — per Ditto's September 2026 review
- Industry average CSR: 92.02%
ICICI Lombard's 3-year average CSR of 84.50% is 7.5 percentage points below the industry average of 92.02%. Ditto's review explicitly states: "its Claim Settlement Ratio of 86.81% falls below the industry average of 92.02%. It's a solid option if you value plan features and pricing, but not the top performer on claims."
PolicyJack.com's independent analysis (March 2026) is more direct: "85.2% CSR (FY24 IRDAI) — roughly 1 in 7 claims does not result in full settlement; this is the most important metric and the primary reason to consider alternatives."
Incurred Claim Ratio (ICR)
Per Ditto's ICR analysis for ICICI Lombard (July 2026): 3-year average ICR of 79.47% vs industry average of 81.88%. The ICR is within the healthy range (55–85%) and close to industry average, suggesting the CSR gap doesn't reflect a pattern of paying very low claim amounts when claims are accepted — it reflects individual claim rejection rates.
Complaint Volume — Better Than CSR Implies
Where ICICI Lombard does better: Ditto's review reports a complaint volume of 15.05 per 10,000 claims — "well below the industry average of 29.35." This means rejected claims are communicated clearly and in a way that doesn't generate complaints — the rejections, when they happen, are based on legitimate policy terms rather than arbitrary decisions.
The Elevate Plan — Features That Stand Out
ICICI Lombard's flagship retail health plan is Elevate, described by Ditto (May 2026) as "a flexible 'build-your-own' comprehensive plan" with 33 optional add-ons.
Features verified from ICICI Lombard's official Elevate page, Ditto's Elevate review (May 2026), and Beshak.org's Elevate analysis (July 2026):
Sum insured: ₹5 lakh to unlimited — the Infinite Care benefit provides lifetime unlimited coverage for a single hospitalisation.
No copayment: Confirmed by Ditto's Elevate review: "No Copayment or Sub-Limits: Elevate skips copayments and disease-wise capping altogether."
No disease-wise sub-limits: No predefined caps on specific treatment categories.
Unlimited Restoration (Reset Benefit): Sum insured restores unlimited times for unrelated illnesses. Per Ditto: "Unlimited Restoration: Sum insured refills for any illness once exhausted, at no extra cost."
Pre-hospitalisation: 90 days — per Beshak.org. This is longer than the 30–60 day standard at most plans.
Post-hospitalisation: 180 days — above the standard 90-day benchmark.
Surrogate and Oocyte Donor Cover — The Standout Feature: ICICI Lombard Elevate covers expenses for surrogate mothers and oocyte donors up to ₹5 lakh. Per Ditto's plan rating: "the plan has a standout Surrogate and Oocyte Cover up to ₹5 lakh." This is genuinely unusual — no other mainstream retail health plan in India provides this coverage, making Elevate specifically relevant for couples pursuing surrogacy or assisted reproduction who want it covered within their main health policy.
Jumpstart Benefit — PED from Day 30: With the Jumpstart add-on, specific pre-existing conditions — diabetes, hypertension, asthma, obesity, coronary artery disease, and hyperlipidaemia — are covered from Day 30 of the policy, per Policybazaar's Elevate page. This is comparable to Aditya Birla's Chronic Care and Care Supreme's Instant Cover.
Power Booster Benefit: 100% cumulative bonus per claim-free year, with no stated ceiling — per Oneassure.in's Elevate analysis.
OPD Coverage (BeFit add-on): OPD expenses covered as an optional add-on — per ICICI Bank's health page.
Modern Treatment Coverage: Per ICICI Lombard's official page: "reward points for healthy habits, video/tele consultation(s), pharmacy and diagnostic services, online chat with doctors, second opinions."
Minimum hospitalisation: 2 hours — Elevate covers hospitalisation of 2 hours or more, vs the standard 24-hour minimum at most plans. Per Policybazaar: "Policyholders can now claim in-patient hospitalization of 2 hours or more under the ICICI Lombard Elevate plan."
Hospital Network
9,500+ cashless hospitals per ICICI Lombard's official Elevate page. PolicyJack.com (March 2026) notes the network at approximately 6,800+ for the older Complete Health product — the Elevate plan appears to have a broader network. For Noida and NCR, major hospitals are likely covered, but this network is smaller than Aditya Birla (16,500+), HDFC ERGO (16,000+), and Star Health (16,000+).

The TPA Concern
Unlike standalone health insurers (which all process claims in-house), ICICI Lombard uses TPAs for cashless authorisation, adding an intermediary layer between the hospital and the insurance decision. PolicyJack.com notes: "TPA-managed claims — unlike Star Health, HDFC Ergo, Niva Bupa, and Care Health (all in-house), ICICI Lombard's cashless process runs through TPAs; adds processing friction."
The digital tracking via the TakeCare app — per Ditto's review — partially offsets this by providing claim status visibility.
Who Should Consider ICICI Lombard Elevate
- Couples pursuing surrogacy or assisted reproduction — the surrogate and oocyte donor cover up to ₹5 lakh is genuinely unique
- Buyers in the ICICI Bank ecosystem who want consolidated financial relationships and are comfortable with the below-average CSR trade-off
- Multi-product buyers who also need motor, travel, and home insurance from the same insurer
- Buyers with diabetes, hypertension, asthma, or obesity who want the Jumpstart add-on for early PED coverage
- Those who want 2-hour minimum hospitalisation to cover shorter procedures that standard 24-hour policies exclude
Who Should Look Elsewhere
- Buyers for whom claim certainty is the primary criterion. At 84.50% 3-year average CSR, ICICI Lombard settles significantly fewer claims than HDFC ERGO (96.71%), Aditya Birla (95.81%), Care Health (93.13%), and Niva Bupa (91.62%)
- Those who want the widest cashless hospital network — 9,500+ vs 16,500+ for Aditya Birla
- Buyers who prefer in-house claims processing without TPA intermediaries
Verdict
ICICI Lombard Elevate has genuinely innovative features — particularly the surrogate cover, 2-hour hospitalisation threshold, and 90-day pre-hospitalisation coverage. The plan has been thoughtfully designed.
The honest concern is the claims data: a 3-year average CSR of 84.50% means approximately 1 in 6 claims doesn't result in settlement. For buyers who will actually claim — which is the entire point of insurance — this matters significantly. Unless the surrogate cover or ICICI ecosystem relationship is specifically relevant to your situation, standalone health insurers with higher CSR offer better primary protection.
